Guide to Filling Out a Passport Application

If you are filling out a passport application through our agency, please prepare the following information:

  1. Passport details, including place and date of birth
  2. Employment history for the last 10 years (including employer addresses)
  3. Current place of residence and registration
  4. Information on surname changes, if applicable
  5. Family status (current marital status)

For a Child’s Passport Application, Provide:

  • Child’s and applicant’s passport details
  • Address of registration for both child and applicant (including registration date)
  • Document proving family relation (e.g., birth certificate)

Documents Required for In-Person Submission at the Consulate (with the Child Present):

📌 For the applicant (parent/legal guardian):

  • Valid identity and citizenship documents (international and, if available, domestic passport)
  • Copies of the passport pages with the applicant’s photo and registration details (if applicable)
  • The child’s birth certificate confirming the applicant’s parental rights

📌 For the child:

  • Original and copy of the birth certificate
  • Internal passport (if the child is 14 or older) with a copy of the photo page and registration page (if applicable)
  • Previous international passport (if issued before) with a copy of the photo page

📌 Additional Documents:

  • A passport application form printed on a single sheet (double-sided)
  • A separate sheet with a barcode, which must be printed along with the application
