Guide to Filling Out a Passport Application
If you are filling out a passport application through our agency, please prepare the following information:
- Passport details, including place and date of birth
- Employment history for the last 10 years (including employer addresses)
- Current place of residence and registration
- Information on surname changes, if applicable
- Family status (current marital status)
For a Child’s Passport Application, Provide:
- Child’s and applicant’s passport details
- Address of registration for both child and applicant (including registration date)
- Document proving family relation (e.g., birth certificate)
Documents Required for In-Person Submission at the Consulate (with the Child Present):
📌 For the applicant (parent/legal guardian):
- Valid identity and citizenship documents (international and, if available, domestic passport)
- Copies of the passport pages with the applicant’s photo and registration details (if applicable)
- The child’s birth certificate confirming the applicant’s parental rights
📌 For the child:
- Original and copy of the birth certificate
- Internal passport (if the child is 14 or older) with a copy of the photo page and registration page (if applicable)
- Previous international passport (if issued before) with a copy of the photo page
📌 Additional Documents:
- A passport application form printed on a single sheet (double-sided)
- A separate sheet with a barcode, which must be printed along with the application
